For the fastest review, include the product name, strength, and lot number when available. Customers can reference their Apexium order number after an order has been placed. The order team will provide the applicable documentation or advise if additional details are needed.

Documentation is evaluated by product and lot. A general product name alone may not identify the correct record, especially where multiple strengths or lots are active. Keep the vial label and packaging information available when making a request.
